How to Redirect Your 1.2%

1. Go to:

deklaravimas.vmi.lt
Click “Prisijungti prie EDS” to log in.

2. Log in securely

Use your internet banking, mobile signature, or another preferred method.

3. Enter the EDS system

On the main page, select the section “VMI sistemos → EDS (Elektroninio deklaravimo sistema)“

4. Start the form

Select: „Deklaravimas → Pildyti formą“

5. Find form FR0512 v.5

Form title:
„Prašymas pervesti pajamų mokesčio dalį paramos gavėjams ir (arba) politinėms organizacijoms“

⚠️ Note: If you had non-employment income (like rental or freelance), submit GPM311 first — then proceed to FR0512.

6. Fill out the form:

  • Recipient: VŠĮ Action4.life

  • Company code: 306070930

  • Percentage: 1.2%

  • Submit it no later than May 1, 2025
